Requesting a Medicare Advantage referral from a primary care provider

As of January 01, 2026, most Medicare Advantage plans now require a referral from primary care providers for specialist services to be covered. In order to facilitate the timeliest submission of these referrals, Boise Family Medicine requests that specialist provider offices, facilities, and hospitals download our Medicare Advantage PDF form by clicking this link.

Once this form is completed and faxed back to our office with documentation supporting medical necessity, our staff will submit the referral to the member’s Medicare Advantage health plan for approval.

Please be aware, the timeline for submission/approval of Medicare Advantage referrals is subject to our office’s staffing limitations and the Medicare Advantage health plans’ ability to process our submissions in a timely manner.

If you are patient of Boise Family Medicine and have been told by a specialist provider office, facility, or hospital that a referral is required from your primary care provider, please call our office at (208) 377-3368 to schedule an appointment with your provider. Your provider and our staff will help facilitate gathering the required information to submit your referral(s) during your appointment.
